Abstract
Die Versorgung von Patienten mit Aortenstenose hat sich mit Einführung des katheterbasierten Aortenklappenersatzes („transcatheter aortic valve implantation“, TAVI) deutlich gewandelt. Nachdem diese Technik initial nur für inoperable Patienten indiziert war, sind heute Patienten mit mittlerem Operationsrisiko auch für diese Intervention geeignet, so ein transfemoraler Zugang gewählt wird. Während Gefäßkomplikationen und postinterventionelle paravalvuläre Leckagen in den letzten Jahren aufgrund technischer Weiterentwicklungen minimiert werden konnten, wird der postinterventionelle Erfolg der TAVI nach wie vor signifikant durch patientenseitige Risikofaktoren beeinflusst. So gilt auch in diesem Zusammenhang das kardiorenale Syndrom als eine der stärksten Risikokonstellationen für den postinterventionellen Verlauf nach TAVI. Die diesbezügliche Datenlage wird erläutert.
With the introduction of catheter based aortic valve implantations (TAVI) the interventional
approach has gained great importance in the treatment of aortic stenosis. After this technique
had started with an indication for high risk patients only, intermediate risk patients are today
also amenable for TAVI as long as the transfemoral approach is chosen. Numerous predictors
have been identified that would lead to periprocedural complications, that on the one hand are
defined by the patients`comorbidity and that on the other hand come along with the technical
approach. While vascular complications and postinterventional paravalvular regurgitation
have been minimized over the past few years by novel technologies, there is a prevailing

individual risk on the patients`side brought about by the specific pathophysiology of the
cardio-renal syndrome that has to be dealt with.
